Nobody reread the list
A patient starts a statin. Their muscles ache. The ache is a side effect, but it reads like a new problem, so it gets treated as one: a pain reliever. The pain reliever brings constipation, which gets a laxative. Nobody has done anything unreasonable. Every decision made sense at the time it was made.
Researchers in Ontario went looking for how often this happens at scale. They took a long list of known “prescribing cascades” and checked each one against the prescription records of 2.3 million people over 66. Twenty-four met all three of the study's criteria for a high-priority, potentially inappropriate cascade: how often the first drug is used, how often the second follows it, and how strongly the two are linked. The most frequent was iron supplement to laxative, hitting about 12 percent of the people who started the iron. Next was statin to pain reliever, about 11 percent. (BMJ, September 2026.)
A cascade is not one bad decision. It is a record nobody reread. Every entry was defensible when it was written. What was missing was a person opening the whole list and asking what it looks like together.
Most of what I do is read other people's work before they show it or fund it. Almost nobody who asks for that needs a new opinion. They have opinions; they wrote the thing. What they need is someone to read the whole list in one sitting and say what it looks like together. Which line is carrying the piece. Which line is only treating a side effect of the line above it.
First read is where you make it. Second read is where you find out what you made. Writers know this about their own drafts, which is why the reliable advice is never “write more” but “put it down and read it again later.” The hard part is that you cannot do it for yourself: you wrote every entry, so every entry is reasonable to you.
That is the job. Not taste. Not a better opinion. Just being the one who reads straight through and says out loud what is on the list.
