• Trial description: three Ontario sites, Jul 2022-Nov 2025, ages 21-65, MADRS 21+, two or more failed trials, four 40-minute infusions, 0.5-0.75 mg/kg ketamine vs 0.02-0.03 mg/kg midazolam, adjunctive meds stable throughout. Holds.
• Withdrawals: five, one ketamine and four midazolam (63/68 in the efficacy analysis). Holds.
• Allocation guess rate: “47% guessed allocation” is 31/66 in the paper. Holds.
• Response comparison: “roughly 55% vs 39%” matches Iovieno 2016 exactly (55.1% vs 39.2%, RR 1.29, 17 trials). Holds.
• Primary result: printed as “between-group difference, -7.3 points; 95% CI, -12 to 2.5.” The paper reads “95% CI, -12.0 to -2.5; P = .003; Cohen d = 0.7.” One dropped minus. As printed, the interval crosses zero, which contradicts the “significantly lower” sentence above it. That is the exact slip that becomes a screenshot saying the trial failed.

Status as of Sep 16: the line prints as written.

Why this exists

A missing minus sign on a confidence interval turns “significantly lower” into a screenshot that says the opposite. Small errors outlive the drafts they came from. The check is cheap; the wrong number in a screenshot is not. That asymmetry is the whole job.
